A number of manufacturers (Like Meris, Empress) implemented TRS MIDI in a way that the devices receives MIDI on the Tip, and sends MIDI on the Ring. The MIDI Standard only specifies MIDI on Tip and Power on Ring to power the opto-isolator.
How is this implemented without forgoing isolation?

For the standard TRS scheme, you cannot say that tip is the signal and ring is power; both tip and ring form the current loop that transmits the signal.
The Meris/Empress scheme forgoes isolation; the optoisolator is in the separate MIDI box. Using a custom adapter that connects only tip or ring might work with some MIDI devices, but cannot be guaranteed to work everywhere. (And neither Meris nor Empress call their TRS cables "MIDI".)
